Isolated Tarsal Navicular Fracture-dislocation
نویسنده
چکیده
Nine cases of fracture-dislocation of the tarsal navicular are reported. Six patients were male and three were female. Their mean age at the time of injury was 30.4 years old. The cause of the injury was a traffic accident in five cases and a fall from a height in four. Open reduction and internal fixation was performed in four cases (three by Kirschner wire, one by screw) and plaster fixation in four. Bony union was achieved in every case, except one chronic case. Arthropathic change was observed in two cases. No aseptic necrosis was found at follow-up. Pain on walking persisted in two cases. None of the patients complained of significant interference with ADL. Fracture-dislocations of the tarsal navicular are very rare, because the tarsal navicular is a small bone that is protected by surrounding bones and tightly bound by strong ligaments. These fractures usually occur in combination with fracture-dislocation ofLisfranc'sjoint, because forces great enough to cause fracture-dislocation of the tarsal navicular cause other fracture-dislocations at weaker sites. Thus, isolated navicular fracture-dislocation is extremely rare. Fractures without significant displacement were treated conservatively by plaster cast. Fracture-dislocations were reduced surgically and fixed internally, because manual reduction was difficult and dislocation was prone to recur.

Tarsal navicular stress fracture in a young athlete: case report with clinical, radiologic, and pathophysiologic correlations.
BACKGROUND Tarsal navicular fractures are uncommon but important causes of foot pain. Being alert to this condition can help prevent a delay in the diagnosis. METHODS A literature search of MEDLINE was undertaken, and a case report of an adolescent with tarsal navicular stress fracture is described. RESULTS AND CONCLUSIONS Tarsal navicular fractures are often misdiagnosed for months. Becaus...
متن کاملTotal dislocations of the navicular: are they ever isolated injuries?
Isolated dislocations of the navicular are rare injuries; we present our experience of six cases in which the navicular was dislocated without fracture. All patients had complex injuries, with considerable disruption of the midfoot. Five patients had open reduction and stabilisation with Kirschner wires. One developed subluxation and deformity of the midfoot because of inadequate stabilisation ...
متن کاملThe non-surgical and surgical treatment of tarsal navicular stress fractures.
Stress fractures of the tarsal navicular, first described in 1970, were initially thought to be rare injuries. Heightened awareness and increased participation in athletics has resulted in more frequent diagnosis and more aggressive treatment. The vascular supply of the tarsal navicular results in a relatively avascular zone in the central one-third, which experiences severe compressive forces ...
متن کاملStress fractures of the tarsal navicular.
Tarsal navicular stress fractures present a difficult diagnostic and treatment dilemma for the orthopedic physician of an active individual. Patients often complain of diffuse, poorly-defined symptoms and have a paucity of physical findings. Initial diagnostic evaluation often. fails to recognize navicular stress fractures which results in delayed diagnosis and treatment. A bone scan is sensiti...
متن کاملOperation for non-union of stress fracture of the tarsal navicular.
Stress fractures of the tarsal navicular do not heal predictably with conservative treatment, so we recommend operation if the fracture remains symptomatic, and radiographs show wide separation of a complete fracture, extension of an incomplete fracture, delayed healing, or a medullary cyst. An autologous bone graft is inserted after en-bloc resection of the fracture surfaces. It is important t...
